Lisa Kristin Forman – 72, succumbed to dementia on Feb. 06, 2026. Lisa was born April 09, 1953, in Glendale California, the daughter of Orman and Gretchen Jacques whom have both passed predeceased.

Lisa graduated from Santa Maria High School, Class of 1971. Following graduation, Lisa married Wesley Forman her high school sweetheart and followed with two beautiful boys, Jeremy S. and Zachary T. and raised them locally in Orcutt CA. Lisa became a local Artist, opened a local coffee shop called Union Street Café in Old Orcutt with her family, and was active in soccer as well as volunteering to be a soccer coach for the local youth community; Lisa also worked as a graphic Artist at Allan Hancock College. Lisa, loved the outdoors – which included hiking, rock climbing, camping, and strolling the beach.

In addition to her parents Lisa leaves her only kin, Michael Jacques and his wife Marilyn, their daughters Elise and Kristine, and grand-daughter Samantha. Then Lisas eldest son Jeremy and wife of 31 years Nena, with four grandkids: Andreana, Savanna, Joshua, and Stuart followed by 10 great-grandkids. Then her younger son, Zachary and his three children: Caleb, Abby and Zoe.

A celebration of life will be held at the Grainge #746 in Grover Beach, on March 14th, 2026 at 370 S 13th St. For those who wish to be part of the celebration, the family will make a farewell at Pismo Beach Pier at 11:30 am, followed by the celebration of Life.
